Supplemental Addenda
Some Forms have addenda and supplemental pages that are necessary to send along with the form itself. Many times in say Real Estate Forms, photos, at least – if not sketches and maps, are required to be sent along with the main form.
Appraisal, Brokerage, Insurance, and Medical forms can have lots of addenda, including photos, sketches, maps, certifications, other documents, etc. Below is the lower portion of a list of Addenda that are available for an Appraisal form.

To include supplemental addenda along with a main form, just mark the ones that are desired. To mark multiple ones at a time, click and drag the mouse or stylus across items. In the image below, the mouse has just been dragged across the desired ones.
